1. Information we collect
Depending on how you use the website, we may collect the following categories of personal information:
| Category | What this includes |
|---|---|
| Contact details | Your name, email address, phone number and, for orders, your billing and shipping address. |
| Inquiry details | The treatment or service you ask about, your preferred appointment times, and anything else you write in a contact form or email. |
| Order information | Products ordered, order totals, pickup or shipping choice, and order history. We do not see or store your full payment card number. Card details are entered into and processed by our payment processor (see Section 7). |
| Account information | If you create a store account: your username, email address, an encrypted (hashed) password and saved addresses. |
| Technical information | IP address, browser type, operating system, device type, the page that referred you, and the pages you request. |
| Usage information | Pages viewed, time spent on pages and similar statistics collected by the analytics tools described in Section 5. |
We do not ask for, and ask that you not send through the website, government identification numbers, financial account numbers, or detailed health information.
2. How we collect it
- Directly from you — when you fill out a contact or consultation form, place an order, create an account, email us or call us.
- Automatically — when you browse the site, through cookies, server logs and the analytics services described in Section 5.
- From service providers — for example, confirmation of a payment from our payment processor, delivery updates from shipping carriers, and order information from Fullscript when you order through our dispensary (see Section 6).
3. Why we use it
- To respond to you — answering questions, scheduling consultations and following up on requests.
- To fill orders — processing payments, preparing orders for in-office pickup or shipping, and handling returns and order questions.
- To communicate with you — sending order confirmations, appointment-related messages and, if you ask to receive them, news and offers. You can unsubscribe from marketing messages at any time.
- To run and improve the website — fixing errors, understanding which pages are useful and improving how the site works.
- To keep the site secure — preventing spam, fraud and unauthorized access.
- To meet legal obligations — including tax and record-keeping requirements and responding to lawful requests.
We do not sell your personal information, and we do not use it to show you targeted advertising on other websites.
4. Health information and HIPAA
Lucencia Medical is a medical practice. When you become a patient, the health information in your medical record is protected under the federal Health Insurance Portability and Accountability Act (HIPAA) and Florida law, and it is handled as described in our Notice of Privacy Practices. You can ask for a copy of the Notice of Privacy Practices at our office or by contacting us using the details in Section 15.
This website Privacy Policy covers information collected through the website itself, such as contact form messages, store orders and analytics. If health information you send us through the website becomes part of your medical record, it is protected as described in the Notice of Privacy Practices.
5. Cookies and analytics
Cookies are small files placed on your device. We and our service providers use them for the following purposes:
| Purpose | Detail |
|---|---|
| Strictly necessary | Keeping you signed in, remembering what is in your cart, completing checkout, and protecting the site and our forms from spam and automated abuse (including Google reCAPTCHA on our contact form). The store cannot work without these. |
| Payments and fraud prevention | Our payment processor uses cookies and device information during checkout to process payments and detect fraud. |
| Analytics | Google Analytics and WordPress.com (Jetpack) statistics help us understand which pages are visited and how people use the site. These reports are about overall site use and are not used to identify you. |
| Embedded content | Maps and other content embedded from third-party services, such as Google Maps, may set their own cookies when they load. |
We do not currently use advertising pixels or cross-site advertising trackers on this website. You can block or delete cookies in your browser settings. Blocking strictly necessary cookies will stop parts of the site, including checkout, from working.
6. Fullscript supplement orders
Some supplements on our site are sold through Lucencia Medical’s dispensary on Fullscript, a third-party online supplement platform. When you select “Checkout at Fullscript,” you leave our website and complete your order on Fullscript’s site. Fullscript collects the information needed to create your account, take payment and ship your order, and its own privacy policy and terms apply to that information.
Because Lucencia Medical runs the dispensary, we may be able to see the orders and product recommendations connected to your Fullscript account so our providers can support your care.
7. Who we share information with
We share personal information only as described below.
Service providers
We use companies that handle information for us, only for the purposes we specify. These include providers in the following roles:
- Website hosting and security — hosting the site and running a content-delivery and security network that processes IP addresses and request data to serve pages and block malicious traffic.
- Online store and payments — running the store (WooCommerce) and processing card payments (WooPayments, powered by Stripe). Card details go directly to the payment processor; we receive only limited details such as the card type and last four digits.
- Shipping — carriers and shipping services receive the name, address and contact details needed to deliver an order.
- Email delivery — sending order confirmations and replies to your messages.
- Analytics and spam protection — as described in Section 5.
- Fullscript — as described in Section 6.
Legal and safety reasons
We may share information when we believe in good faith that it is required by law, regulation, subpoena or other legal process, to respond to a lawful request from a government authority, to enforce our policies, or to protect the rights, property or safety of Lucencia Medical, our patients, customers or others.
Business transfers
If Lucencia Medical is involved in a merger, sale of the practice or similar transaction, personal information may be transferred as part of that transaction, subject to protections consistent with this policy and applicable law.
8. How long we keep it
We keep personal information only as long as we need it for the purpose it was collected, and longer when the law requires it or when it is needed to resolve disputes. In general:
- Contact form messages — kept while we are responding to you and for a reasonable period afterwards.
- Order records — kept for the period required by tax and accounting rules.
- Medical records — kept for the period required by Florida law, as described in our Notice of Privacy Practices.
- Analytics data — kept according to each analytics provider’s retention settings and then deleted or combined into overall statistics.
- Marketing contacts — kept until you unsubscribe or ask us to remove you.
9. How we protect it
We use administrative, technical and physical safeguards designed to protect personal information, including encrypted (HTTPS/TLS) connections, limited access to customer records, a managed hosting environment with a web application firewall, and established providers for payments. No method of sending or storing information is completely secure, so we cannot guarantee absolute security.
If a breach of security affects your personal information, we will notify you as required by Florida law (section 501.171, Florida Statutes) and, for health information, by HIPAA.
10. Your choices and rights
Whatever state you live in, you can ask us to:
- See the personal information we hold about you from your use of the website;
- Correct information that is wrong;
- Delete information, unless we need to keep it for legal, medical-record, tax or order reasons;
- Stop marketing — unsubscribe using the link in any marketing email, or contact us.
Patients also have rights over their medical records under HIPAA, such as the right to see and get a copy of their records and to ask for corrections. Those rights, and how to use them, are explained in our Notice of Privacy Practices.
To make a request, contact us using the details in Section 15. We will confirm your identity before acting on a request, for example by checking that you control the email address or phone number connected to the information. We will not treat you differently for making a request.
11. Phone calls and messages
When you call us, our staff may make notes of the call so we can help you. Florida law requires the consent of everyone on a call before it is recorded. If we ever record a call, we will tell you at the start of the call. If you contact us by email or text, we keep those messages as described in Section 8.
12. Children
This website is intended for adults. Our online store is for customers 18 and older, and we do not knowingly collect personal information online from children under 13. If you believe a child has given us personal information through the website, please contact us and we will delete it.
13. Third-party sites and services
Our website links to, and embeds content from, other services, including Fullscript, Google Maps and social media sites. Those services have their own privacy policies, and this policy does not cover their practices. Please review their policies before giving them your information.
14. Changes to this policy
We may update this policy from time to time. When we do, we will change the “Last updated” date at the top of this page. If we make a significant change to how we use personal information, we will post a more noticeable notice on the website.
